Health Information


When a student is absent from school, a parent should call the school by 9:00 AM. In addition, a parent should send a note to school upon the student’s return, stating the dates and reason for the student’s absence.

Prescriptions : Prescriptions and medications, including over the
                          counter medications, to be taken at school must be
                          in their original container, accompanied by the
                          Connecticut State authorization form signed by
                          both the doctor and the parent. All over the counter
                          medications must be brought in unopened. A      
                          parent or guardian must bring the medication to
                          the nurse. Students are not allowed to carry
                          medications to school.        

                              
Immunization Requirements Grade 5 & 6:
1. A measles vaccine booster
2. A completed Hepatitis B vaccine series
**If these requirements are not met, your child will not be allowed to start his/her school year.



Physical Examination requirements for grade 6:
All 6th grade students are required to have a physical exam. This may be done from January 1st of 5th grade through December 31 of 6th grade. This is a calendar year.
If this requirement is not met, your child will not be
allowed to return to school after the Christmas break. You can go to the web site below to obtain the State of Connecticut Department of Education Health Assessment form or you can get one from the school nurse.
